You can make a rip, if you have separated game and patch files
You need next things: ps4pkg viewer, clonespy, gengp4, orbis-pub-gen.
Let's make a Rip of Killing Floor 2 for example.
Open the game file with ps4pkg viewer and remember APP_VER and VERSION, extract this pkg to any folder you want and name it CUSAXXXXX.
After that move everything from the Image0 folder to CUSAXXXXX, from Sc0 to sce_sys. Open the param.sfo file and write there correct APP_VER and VERSION.
Do the same thing with the patch file, but folder must be named as CUSAXXXXX-patch
Open the CloneSpy, add the KFGame folder to the Pool 1 from the CUSAXXXXX and KFGame to the Pool 2 from the CUSAXXXXX-patch, set Compare Pool 1 to Pool 2 mode, search pools for duplicates, duplicates with the same file name and titles, files having the same name and title, delete all the results from the Pool 1.
Now open the gengp4, generate and save .GP4 files of both CUSAXXXXX and CUSAXXXXX-patch folders. Open Orbis Pub Gen and make a fpkg of CUSAXXXXX.GP4 with it. Don't forget to choose the created game pkg as Application package in project settings during creating the patch of CUSAXXXXX-patch.
